Sabbatical Report Form
Upon completion of a sabbatical leave, a report is due that describes in detail the work of the sabbatical and the accomplishment of the specific goals stated in the sabbatical proposal. Also due is a one-page executive summary of this report that will be published for the faculty and given to the board of trustees.

Please complete the form in its entirety. You will not be able to save the form and come back to it. This electronic report precludes the need to submit paper copies of your report and executive summary to the Vice president for Academic Affairs and Dean of the College and the chair of the Committee on Professional Development. These requirements are complete when you submit this form and will become part of your permanent file in the office of the Vice president for Academic Affairs and Dean of the College. Please note that this report will be considered in subsequent decisions concerning such matters as promotions, tenure, and salary. In addition, the report will be considered by the Committee on Professional Development when subsequent requests for grants, fellowships, and sabbaticals are requested.
